IMPORTANT regarding all 2020 Prepaid Recreation Membership & Locker Rentals

Submit this form to request a prorated refund for the remaining balance since our closure on March 16, 2020. Membership and locker rental extensions or credits are not available.

Refunds for non-UCLA employees will be issued as a paper check and mailed to the mailing address listed on this form. Due to the high volume of requests, checks will be mailed within 12-15 business weeks after request is submitted. Current UCLA employees will be issued a refund via direct deposit within 5-10 business weeks.

  • Transfers: If transferring, the fee previously paid for activity #1 will be applied toward activity #2. Any due balance must be paid when completing this transaction with the Sales & Service team. If there is a balance due back to you, the money will be refunded per the refund policy schedule.
  • Cancellations: Notification of all refund denials will be sent to the email address provided on the front of this form.
  • Refunds: Submitting this form does not guarantee a refund.  Refunds for services originally paid in the last 6 months with a debit card, credit card or Bruin Card normally post within 5-7 business days.

Membership Cancellation - Refund Request

We are experiencing a nationwide pool chemical shortage that affects our ability to increase access to the pools as well as social distancing requirements in facilities, our capacities remain limited.  Therefore, UCLA Recreation is unable to extend or reactivate any prepaid 2020 memberships as we had planned.  

Please submit this form to initiate your membership and locker cancellation and refund process.  All memberships paid for via Payroll Deduction have been cancelled effective March 16, 2020. No action is necessary and no refunds are offered.

Locker Rental Cancellation

We are no longer accepting locker rental HOLD requests as of September 17, 2021. All remaining lockers have since been cancelled and cleared.
